Improving Diabetes Outcomes with a Medicaid Health Plan: How Scene's 360° Model of Care Helped Improve Medication Adherence, Decrease AlC Levels, and Achieve High Program Satisfaction

A Maryland Medicaid Managed Care Organization (MCO) that covers over 50,000 individuals, was looking to improve medication adherence among its diabetic members who were diagnosed with persistent high blood sugar (HbA1c > 7) despite being prescribed up to three oral anti-diabetes medications. 

In this case study, we show how we partnered with the MCO to help over 200 of its members achieve increased oral diabetes medication adherence rates, improved medication possession ratios, and high program satisfaction rates. In today's value-based environment, adherence solutions like ours represent a sound investment for health plans charged with prioritizing the health of their populations, improving quality of care, and lowering healthcare spending.
